  • 19-100 years of age.
  • patient presenting to the UAB Highlands Pain Treatment Clinic for initial evaluation and treatment or subsequent follow-up evaluation and treatment.
Exclusion Criteria
  • <19 or >100 years of age
  • a current prisoner
