1. Adequate mouth opening( > 30mm)
2. Impacted mandibular third molar present with similar extraction difficulty index as per Pederson difficulty index (mesioangular and vertical) and class I and class II with position A and position B according to Pell and Gregory classification. Which need to be removed surgically.
3. Patients belonging to ASA I(American society of anaesthesiology) category.
4. Patients who are willing to undergo the surgical procedure of transalveolar extraction
1. Systemic diseases (such as diabetes mellitus, hypertension, bleeding disorders, any autoimmune diseases)
2. Pregnant patients and lactating mothers
3. Teeth affected with acute infections, such as pericoronitis, an acute alveolar abscess at the time of presentation
4. Patient allergic to local anaesthesia.
5. Patient undergoing radiotherapy and chemotherapy.
6. Alcoholic patients and patients with smoking habits
7. Patient allergic to penicillin or other drugs which will be used in the standardized postoperative therapy.
