- •1.Diagnosed case of diabetes within the age group 18 years & above.
- •2.Patients who are willing to participate.
- •3.Patient receiving insulin therapy for treatment of diabetes.
- •4.Diabetic patients with more than two records of HbA1c outside of recommended range >7% and HbA1c.
排除标准
- •1.Those who are not able to communicate regularly on phone.
- •2.Patient with gestational Diabetes.
- •3.Patient with serious or life threatening illness.
- •4.Patient involved in separate trial.
