There were 18837 patients presenting to the ED from RACFs. 64% were female with an average age of 87 years. 92% arrived via ambulance and 45% were admitted to hospital. The incident rate ratio of RACF admissions per month was 0.79 meaning that residents were 21% less likely to be admitted to hospial. 7 day ED reporesentations dropped from 5.7% to 4.9% and 30 day readmissions was similar at 12% in the control period and 10% in the intervention period.

- Primary Outcome Measures
Name Time Method Admission to hospital. Hospital administrative data was used to identify ED transfer and hospital admission. [There was a 3 month transition period after the intervention was introduced to each RACF. Comparisons were made comparing admissions to hospital for each RACF before and after the intervention per one thousand RACF bed days per month. ]
