randomly selected new or follow up OPD cases from the patient of different departments like Medicine, Surgery, Psychiatry, Pediatrics, Obstetrics and gynecology, Skin, Dental, Ophthalmology, Pulmonary Medicine, ENT and General OPD, who visited Central Pharmacy Store for medicine and all the necessary information regarding prescription were filled in to coded case record form. Average 30 to 35 case record forms were filled per week. Every time different day of week was selected to get various prescriptions from different unit of departments.
