- •1)H/o of bleeding disorders
- •2)H/o Hypertension
- •3)H/o Smoking
- •4)H/o Alcoholism
- •5)H/o Seizures
- •6)H/o allergy, hypersensitivity or reaction to any medication
- •7)H/o receiving chronic medication for any type of illness
- •8)H/o decreased visual acuity without correction
- •9)Pregnant and lactating women
