- •Patient is being treated by a participating doctor
- •- Patient receives home visits from a care assistant
- •- Conducted patient information with voluntary and written consent for the pseudonymized recording of health-related quality of life and patient satisfaction as well as for the anonymized recording of the care provided and the processing of the personal address by the responsible assistant or a person named by the assistant to calculate the route
- •- Patient is mobile enough to visit the VaO room (sufficient mobility according to the assessment of the supervising general practitioner)
- •- Sufficient knowledge of German
