[Patient]
1. Patients diagnosed with heart failure by a cardiologist and receiving ongoing outpatient follow-up
2. Patients who are alert and able to communicate
3. Patients who understand the purpose of the study and agree to participate in the study
[Caregiver]
1. Caregivers who have been involved in the care of a patient with heart failure as the primary caregiver for at least 6 months
2. Caregivers who are alert and able to communicate
3. Caregivers who understand the purpose of this study and agree to participate in the study

1. Patients with a history of stroke, Alzheimer's or vascular dementia, memory impairment, and psychiatric disorders
2. Patients with comorbidities such as chronic kidney failure and end-stage cancer
[Caregiver]
1. Caregivers with end-stage cancer, cognitive impairment such as dementia, psychiatric history including depression, or difficulty completing questionnaires
