The apical third of the root canal is considered to be a critical zone because of presence of large number of ramifications and lateral canals in this region.Recognition of microorganisms in the apical region of root canal has led to the acceptance of enlargement of apical diameter that allows for better penetration of irrigants, thus, optimizing the root canal debridement and disinfection. However, the extent of this enlargement is still a debatable topic.The purpose of this study is to evaluate the effect of apical sizeenlargement and taper on post-operative pain and the outcome of primaryendodontic treatment.
The present studywill be prospective clinical study conducted in the Department of Conservativedentistry and Endodontics, Dr. Ziauddin Ahmad Dental College and Hospital. Thestudy will involve two visit root canal treatment with different apicalenlargement size and taper of the file system used. Post-operative pain will beevaluated using VAS scale and outcome will be evaluated based on periapicalindex (PAI) scored on periapical radiograph.

Mandibular first molar with periapical radioluscency.
Patients who are pregnant Patients who are uncontrolled diabetic Patients who areimmunocompromised Previously accessed root canal system with procedural errors.
