Inclusion Criteria
i) Systolic heart failure with documented history of left ventricle ejection fraction (LVEF) < 40%
ii) If most recent evidence of LVEF > 40%, NYHA to be > Class I
iii) Optimal Heart Failure medical therapy that includes a beta-blocker and ACEI or ARB
iv) Able to read and write in Kannada
Exclusion Criteria
i) Atrial fibrillation or implanted rhythm or pacing device
ii) < 3 months since heart failure admission, major illness or surgery, severe psychological stressor
