- •Age 18 years or older
- •Confirmed diagnosis of hypertension
- •Taking at least one antihypertensive medication

- •Have a diagnosis of cognitive impairment or serious medical condition as determined by their primary care physician
- •Unable to provide informed consent or refuse to participate
