Inclusion Criteria
- •Patients who had had at least 3 months of onset of hemodialysis and had had an acute illness
- •Patients treated with hemodialysis three times a week and 3-4 hours each
- •Patients who approved their initial consent and one of their family members to participate in the study
- •Patients who were able to speak and understand fully the Persian language, and at least to read and write literacy.
- •Patients in the age range of 18-65 years
- •Patients who had not already received the home visit plan
Exclusion Criteria
- •Patients with acute and chronic infections.
- •Patients with malignancy.
- •Patients with cognitive and psychological problems.
