1. age 35 and over, both male and female 2. diagnosed by a doctor with high blood pressure, diabetes mellitus type 2, hyperlipidemia, chronic kidney disease, stroke, and ischemic heart disease for at least one year 3. is a person whose name appears in the Phayao Province civil registry 4. no mental or physical disorders such as dementia or paralysis, and 5.voluntarily participate in research program.
1. aged 18 years and older, both male and female 2.) be a family member and care for an elderly person in your household for at least a year 3. can read and write Thai, and have no hearing issues and 4. voluntarily participate in research program.
