Inclusion Criteria
Patients on 5 or more prescribed drugs with at least one FRIDs
Ambulatory
Willing to participate & give consent
MoCA (Montreal Cognitive Assessment) with a score above 26 and can walk 10 meters or with a walking aid(walking stick, canes, callipers)
Exclusion Criteria
Critically ill patients,
Patients with known neurological illnesses, cognitive impairment/ Dementia, Parkinsonism, stroke
Elderly on homecare or special care
Older adults using walkers, wheelchair

Name Time Method Frequency of falls Assessed by using Fall history questionnaire of McCrum 2020and fall efficacy scale <br/ ><br> <br/ ><br>Fall risks will be assessed by TUG & POMA <br/ ><br>Timepoint: Assessment at the base line & every 3 months till 1 year
