Eligibility Criteria

Inclusion Criteria

  • A. Meet the diagnostic criteria of stage ? and above of type 2 diabetes; B. Meet the diagnostic criteria of diabetic retinopathy; C. Signed informed consent; D. Ages 35 to 75\.

Exclusion Criteria

  • A. Associated with other eye diseases: optic atrophy, iridocyclitis, uveitis, macular degeneration, glaucoma; B. Patients with severe heart failure or other serious physical diseases who are unable to take care of themselves; C. People with cognitive and communication disabilities; D. Outpatient treatment without hospitalization; E. Those who disagree with the review.

Intervention to implement care monitoring for people living with HIV / AIDSThis project is an implementation study that aims to test an intervention to improve the monitoring of health care continuum (MCC) to people with HIV infection linked to SUS services in the state of São Paulo. The hypothesis is that the intervention will be able to intensify and improve the current monitoring activities in order to provide improvements in the care processes of PLWHA and the incorporation and use of the Clinical Monitoring System (SIMC) by the health services involved in the project.
