Decompensated heart failure with LVEF <40%. Potential participants must meet the standard
definition of heart failure based on the European Society of Cardiology Criteria. Echo-cardiography based assessment of LVEF is an essential criteria. Heart failure probability will be assessed based on clinical history,
physical examination and ECG abnormality. Assessment of natruretic peptides will not be an essential criteria.
Bed-ridden patients and those who are not able to provide written informed consent.

Name Time Method The number of days alive and out-of hospital (DAOH) during the two-year follow-up period.
