1.Age of 65 year old and older
2.Admitted to SICU at Siriraj hospital within 7 days following surgery and 3.anticipated length of stay more than 24 hours
4.Richmond Agitation Sedation Scale (RASS) at admission between 1 to -2
5.Can communicate in Thai
1.Severe visual or auditory impairment
2.Pre-existing cognitive dysfunction
3.the Confusion Assessment Method for the ICU (CAM-ICU) at admission positive
4.Immobilized or paralysis patient
5.Patient who received neuromuscular blocking agent continuous intravenous drip.
