1.Age, more than 18 years..
2.Undiagnosed exudative pleural effusion.
3. ADA between 40 and 60
1.Respiratory insufficiency, with hypoxemia and hypercapnia
2.Severe chronic obstructive pulmonary disease
3.Pleural thickening of unknown etiology
4.Suspected mesothelioma where the visceral and parietal surfaces fuse
5.Previous pleurodesis
6.Unstable cardiovascular status
7.Known primary malignancy
8.Pleural adhesions
9.Transudative pleural effusion
10.coagulation defects/bleeding diathesis/thrombocytopenia
11.Drug hypersensitivity
12.Severe pulmonary fibrosis.
13.Patient on anticoagulant therapy
14.Inability to lie in lateral decubitus position
