Primary Outcome Measures
NameTimeMethod
The primary endpoint is drug-related rehospitalization.

Whether the rehospitalization is drug-related or not is decided by an expert panel of three pharmacists. Based on the physicians' letters, they can classify the patient's rehospitalization in three categories: drug-related, not drug-related or questionably drug-related. If the panel of experts decides that the rehospitalization is questionably drug-related, other three pharmacists will assess the case according to the three categories.

The primary endpoint is assessed for each participant during the follow-up 30 days and 90 days after discharge from hospital.
Secondary Outcome Measures
NameTimeMethod
The following secondary endpoints will be assessed: total number of rehospitalizations, duration of rehabilitation in days, number of days spent at home, increase in the degree of care, institutionalization, and mortality.

With regard to secondary endpoints, participants will be contacted by telephone during the follow-up 30 days and 90 days after discharge.
