Inclusion Criteria
Patient of ward 17.1 of the University Hospital Cologne
- Age = 65 years
- Multimorbidity (= 2 chronic diseases requiring treatment)
- Patient or proxy able to give consent
- Sufficient knowledge of German
- Degree of care <4
Exclusion Criteria
- Age < 65 years
- Lack of multimorbidity (< 2 chronic diseases requiring treatment)
- Patient or proxy not able to give consent
- Lack of German language skills
- Degree of care =4

- Primary Outcome Measures
Name Time Method The primary endpoint is drug-related rehospitalization.<br><br>Whether the rehospitalization is drug-related or not is decided by an expert panel of three pharmacists. Based on the physicians' letters, they can classify the patient's rehospitalization in three categories: drug-related, not drug-related or questionably drug-related. If the panel of experts decides that the rehospitalization is questionably drug-related, other three pharmacists will assess the case according to the three categories.<br><br>The primary endpoint is assessed for each participant during the follow-up 30 days and 90 days after discharge from hospital.
- Secondary Outcome Measures
Name Time Method The following secondary endpoints will be assessed: total number of rehospitalizations, duration of rehabilitation in days, number of days spent at home, increase in the degree of care, institutionalization, and mortality. <br><br>With regard to secondary endpoints, participants will be contacted by telephone during the follow-up 30 days and 90 days after discharge.
