Eligibility Criteria

Inclusion Criteria

  • No prior exposure to the tool (through development and beta testing)
  • Access to an organized diabetes pathway consistent with the Sanitas EPS (Entidad Promotora de Salud) diabetes mellitus 2 guideline.

Exclusion Criteria

  • Terminally ill
