Eligibility Criteria

Inclusion Criteria

  • Inclusion criteria Type 2 diabetes Years of disease: \>\= 6 months Elevated hemoglobin A1C (\>7 %)

Exclusion Criteria

  • Patients who have a diagnosed psychotic disorder, disabling sensory impairment, or life expectancy of \<12 months, pregnant, active cardiovascular disease, end stage renal failure, breathing problems requiring hospitalization or oxygen use in the previous six months
