Scheduled allogeneic stem-cell transplantation at the University Hospital Freiburg, = 18 years, able to speak, read and write German, able to plan and perform self-management activities independently; WLAN or mobile data access , mental, physical and psychological abilities and health to provide a written informed consent about study participation and related data protection
Patients who do not fulfill the inclusion criteria. Patients with a second allogeneic stem-cell transplantation within the first year after the first stem-cell transplantation will be excluded.

Name Time Method Total healthcare utilization costs in the first year after allogeneic stem-cell transplantation
- Secondary Outcome Measures
Name Time Method re-hospitalization rate, length of re-hospitalizations, rate of medication non-adherence, treatment burden, health-related quality of life, rate of Graft-versus-Host-Disease episodes, overall survival rate and re-hospitalization-free survival rate in the first year after allogeneic stem-cell transplantation.<br>The evaluation of implementation outcomes as acceptability, feasibility, adoptability and fidelity will be done also.
