Subjects will be screened based on inclusion and exclusion criteria from various parts of Belagavi City.Before participating in the study, participants will be told of its objective and will be required to sign a written informed consent form.Data will be collected over a six-month period, with a systematic evaluation capturing demographic information, medication use, and medical history. The study will use validated outcome measures to assess physical fitness and quality of life, such as the FTSST, 6MWT, LEFS, Chair Sit and Reach Test, and KDQOL-36 questionnaire. After collecting the data, the goal is to provide insights into how polypharmacy affects physical fitness and quality of life.

- •Both male and female elderly 65 and above Score ≥24 on the Mini Mental State Examination Dialysis patients with CKD Patients receiving dialysis for at least 3 months, minimum of 25 sessions 30min after dialysis all the outcome measures will be assessed elderly using ≥5 prescribed medications concurrently Patient able to walk with or without walking aids for 100mts.

- •Individuals who have undergone surgery or been hospitalized in the past three months.
- •Lower limb amputations Severe visual or hearing loss Patients with neurological conditions, such as stroke,ALS,MS; Orthopedic conditions, such as severe osteoarthritis,recent fractures ; Diagnosed Unstable angina,post-CABG,pulmonary embolism,myocardial infarction.
