Eligibility Criteria

Inclusion Criteria

  • Patients of ASA physical status III and IV with BMI \>\= 30 kg/m2 scheduled to undergo elective cardiac surgery with fast track extubation will be included in this trial.

Exclusion Criteria

  • 1\)Heart failure
  • 2\)Vasopressor support
  • 3\)Severe Pulmonary Hypertension
  • 4\)Preoperative atelectasis
  • 5\)Preoperative pulmonary disease including \- tuberculosis, pleural effusion, restrictive lung disease, COPD , Asthma
  • 6\)Contraindication to HFNO including blocked nasal passage / choanal atresia , Trauma / surgery to nasopharynx
  • 7\)Negative consent/refusal
