Having an age of over 60 years
Lack of speech, hearing, and vision disorders
Ability to communicate
No history of psychiatric disorders according to the accompanying patient
The absence of employment of a patient or a family member in the medical sciences group
Resident of Kashan city
Obtaining a score of 8 or higher on(AMTS) (Abbreviated Mental Test Score)
Scoring 6 or less on the elderly depression scale(GDS-11) (Geriatric Depression Scale )
Patient's refusal in the middle of the study to participate in the trial
Patient's death
Rehospitalization
Performing (coronary artery bypass grafting) (CABG)

- Primary Outcome Measures
Name Time Method Treatment adherence, which is assessed using the Modanlo questionnaire, examines the following domains: commitment to treatment, ability to participate in treatment, ability to adapt, integrating treatment into life, adherence to treatment, commitment to treatment, and doubt in implementing treatment. The questionnaire score ranges from 0 to 200, and the higher the score, the greater the treatment adherence.. Timepoint: Before the intervention, on the day of discharge, one month after discharge, two months after discharge, three months after discharge. Method of measurement: Modanlo Treatment Adherence Questionnaire.
