- •EPC clients who have an advanced diagnosis of dementia or any non-malignant chronic diseases such as:
- •Cardiovascular
- •Diabetes and its complications
- •End-stage liver disease
- •Haematological
- •Kidney Failure
- •Motor Neurone disease
- •Multiple organ failure
- •Neurological Disease
- •Other Urological
- •Respiratory Failure
- •Other terminal illnesses
排除标准
- •Clients who are:
- •- under 18 years of age
- •- at the terminal palliative care phase
- •- without any carer
