Inclusion Criteria
EPC clients who have an advanced diagnosis of dementia or any non-malignant chronic diseases such as:
Cardiovascular
Colorectal
Diabetes and its complications
End-stage liver disease
Haematological
Kidney Failure
Lung
Motor Neurone disease
Multiple organ failure
Neurological Disease
Other Urological
Prostate
Respiratory Failure
Sepsis
Stroke
Other terminal illnesses
Exclusion Criteria
Clients who are:
- under 18 years of age
- at the terminal palliative care phase
- without any carer
