1.Age > 18 years
2.48 hours to 96 hours post extubation after prolonged intubation
3.All tracheostomized patients who are already on oral feeds or in whom initiation of oral feeds is planned
4.Conscious and able to follow verbal commands [GCS at least E4 VT M6 (10T)]
5. Hemodynamically stable patients
1. Presence of pre-existing dysphagia
2. Severe agitation and possible inability to
cooperate with the examination
3. Bilateral obstruction of the nasal passages
4.Pre-existing neurological conditions, such as
stroke and neuromuscular disease like myasthenia gravis, Lambert-Eaton myasthenic syndrome [LEMS]
5. Conditions affecting anatomic structures of the swallowing tract
6.History of recent trauma to the nasal cavity or surrounding tissue
7. Recent MI < 1month
8. Pre-existing congestive heart failure
9. Cardiac disorder with acute risk of vasovagal episode and bradycardia
10. Severe bleeding disorders and/or recent severe epistaxis (nosebleed)
11. Requiring oxygen more than 5lt/min
12.Recent trans-oesophageal echocardiography
13. Head and neck or oropharyngeal cancer

Name Time Method 1. To identify the prevalence of swallowing dysfunction in post extubated and <br/ ><br>tracheostomised patients without pre-existing dysphagia <br/ ><br>2. To identify the risk factors associated with the development of dysphagia <br/ ><br>3. To correlate between the severity of dysphagia and the its effects on patient outcomes <br/ ><br>Timepoint: At the time of bronchoscopic assessment of swallowing
