- •TSH <0.4 mE/L, FT4 >25 pmol/l, normal or raised T3 , thyroid peroxidase antibodies <50 kU/l, TBII < 2.0 kU/l, amiodarone use, poor or no visualisation of thyroid gland on 99mTc-pertechnetate scintigraphy, no nodular goiter on ultrasound.

- •Very critical illness, drug or alcohol abuse, pregnancy, no informed consent.
