- •1.prior visit to the operation theatre
- •2.refusal by patient
- •3.Impaired eyesight and hearing
- •4.History of psychiatric/neurological disorder
- •5.History of head injury
- •6.History of drug abuse
- •7.History of alcohol abuse
- •8.Any psychological trauma in past 6 months
