- •Known coronary artery disease
- •Good left ventricular function
- •Sinus rhythm

- •Permanent pacemaker
- •Significant valvular heart disease or other structural heart disease
- •Type 1 diabetes
- •Treatment with insulin, exenatide, sitagliptin or vildagliptin
