1. Major depressive disorder; bipolar disorder according to the MINI;
2. Cognitive impairment or dementia;

- Primary Outcome Measures
Name Time Method 1. Primary clinical outcome is the cumulative incidence of DSM-IV major depressive disorder after 12 months using the Mini International Neuropsychiatric Interview (MINI);<br /><br>2. In the economic evaluation, health care utilization and absenteeism and presenteeism will be measured using the most recent update of the TiC-P questionnaire.
