7 question survey evaluating the level of intensity of exercise of individuals with Parkinson's Disease using a Likert scale from 0-5 where 5 indicated higher levels of intensity.

Number of participants reporting 1. less than 12 years of school, 2. completed high school, 3. completed bachelor degree, 4. completed graduate/advanced degree.
