Inclusion Criteria

1. duration of mechanical ventilation more than or equal to 48h

2. age more than or equal to 18

3. clinical suspicion of VAP based on two or more of the following criteria :

A. body temperature > 38 C or < 36 C

B. white blood cells > 10000 or < 4000

C. purulent tracheal secretions

D. PaO2/Fio2 < 300

Exclusion Criteria

ongoing pneumonia

subcutaneous emphysema

thoracic dressings

pregnancy

Primary Outcome Measures

In patients with clinical suspicion of ventilator associated pneumonia(VAP) , diagnostic performance of combination of lung ultrasound plus S.procalcitonin <br/ ><br>for diagnosis of VAP will be assessed by sensitivity, specificity , positive predictive value and negative predictive value and compared with white blood cells, procalcitonin , C-reactive protien or Clinical pulmonary infection score alone.Timepoint: only those patients who are on mechanical ventilation for than or equal to 48hours with clinical suspicion of ventilator associated pneumonia will be included in the study . <br/ ><br>baseline test that will be done at inclusion of patient in the study.no further follow up is required: <br/ ><br>lung ultrasound <br/ ><br>procalcitonin <br/ ><br>CRP <br/ ><br>Tracheal aspirate for culture and stains <br/ ><br>chest x ray <br/ ><br>chest CT ( ONLY IF ADVISED FOR TREATMENT) <br/ ><br>White blood cells <br/ ><br> <br/ ><br>
