Eligibility Criteria
Inclusion Criteria
- •Be registered in the Family Health Units (USF) included in the study with a diagnosis of Type 2 Diabetes Mellitus; of both sexes; older than 18 years
Exclusion Criteria
- •Have a health problem that makes verbal communication difficult/impossible and/or traveling to the Family Health Units; not have telephone contact; change of coverage area of ??the Family Health Units during data collection
