Eligibility Criteria
Inclusion Criteria
- •1\. Consenting adults (age \= 30 years) attending the primary clinics diagnosed with hypertension within one year from the first day of data collection.
- •2\. The individual should have resided in the same locality in the preceding six months and is expected to stay in the same locality for the next six months from the day of baseline\-data collection.
Exclusion Criteria
- •1\. Hypertensives aged less than 30 years of age.
- •2\. Hypertensives diagnosed more than one year ago.
- •3\. Hypertensives with end organ damage at the time of baseline\-data collection
