- •- No possible cardiac surgery or angioplasty
- •- NYHA class II-IV
- •- Left ventricular ejection fraction <40%
- •- Collateral circulation and/or hibernating myocardium

- •- Previous neoplasia (except in situ cervical cancer or non-melanoma skin cancer)

- •- Uncontrolled hypertension
- •- Allergy to G-CSF or EPO
