family physician office visitors, older than 65 years, who take at least one regular medication
Exclusion Criteria
Terminal disease (less than one year life expectancy), dementia severe enough to render patient unable to answer questionnaire

Name Time Method Secondary endpoints are polypharmacy and medication adherence, measured one year after the medication review, and physician visits and unplanned or urgent clinical specialist visits or hospitalizations.
