Inclusion Criteria

1. acute stroke (in the sense of cerebral ischemia, subarachnoid hemorrhage or intracerebral hemorrhage) or TIA with consecutive inpatient treatment at the Neurological Clinic of the University Hospital of Würzburg.
2. age >= 18 years
3. written consent of the patient or legal representative to participate in the study

1. age < 18 years
2. no acute stroke or TIA

Primary Outcome Measures

Through the implementation of CAEHR, an improvement in stroke care, operationalized by an improvement in the degree of fulfillment of specific, standardized quality indicators of cross-sectoral stroke treatment (key performance indicators), will be achieved.
Secondary Outcome Measures

The Secondary Endpoints include:<br>- Added value for healthcare providers (process evaluation) operationalized by:<br>- Reduction of incorrect documentation (e.g., patient identity, date of birth).<br>- Ability to communicate with patient contacts prior to arrival at stroke center.<br>- Time savings in documentation for research purposes<br>- Time savings regarding patient documentation and duration of imaging transfer<br>- Added value for researchers (process evaluation) operationalized by:<br>- Timely access to data<br>- Higher quality of research data<br>- Availability of data<br>- Faster possibilities to expand data collection<br>- Added value for the health care system (health economic evaluation)<br>
