Eligibility Criteria
Inclusion Criteria
- •Usage of a selfdeveloped checklist criteria for possible need for rehabilitation on the level of medical clinics (general practioner) where exactly is defined, what is a positive result of the checklist.
- •In case of positive checklist the patients will be informed and advised on the study and after their aggreement they will be send to the medical clinic with specialty services for further ICF\-baesd assessment und developing an individuell rehabilitation plan.
Exclusion Criteria
- •negativ checklist criteria for possible need for rehabilitation; patient does not agree on participation
