A study on diaphragm ultrasound used for weaning mechanically ventilated patients

- Brief Summary
Total of 200 patients who meet the inclusion criteria will be included in the study. They will be disconnected from the ventilator to Spontaneous Breathing Trial (SBT) for 1 hour with oxygen supplementation. RSBI will be recorded after SBT for 1 minute by the attending physicians who are not involved in this study. RSBI [ respiratory rate/ tidal volume] of less then 105 can predict weaning success from mechanical ventilation. Simultaneously bed side diaphragm ultrasound will be performed at the time of SBT by an experienced anesthesiologist in ICU. On B- mode, Diaphragm thickening fraction [DTF] will be calculated. DTF of more than 35% predicts successful weaning. Then using M- mode Diaphragmatic excursion (DE) will be measured. DE of more than 1.35 cm predicts successful weaning.
The sensitivity, specificity, positive predictive value, negative predictive value of DTF and DE will be evaluated.
The comparison between DTF and RSBI will be studied by measuring area under Receiver Operating Characteristic Curve (ROC) curve.
Similarly, the comparison between DF and RSBI will be studied by measuring area under Receiver Operating Characteristic Curve (ROC) curve.

Age 18 to 60 years patients on mechanical ventilation for more than 24 hours Ability to tolerate SBT for 1 hour before ultrasound stable hemodynamics conscious and oriented minimal tracheobronchial secretions effective cough response.
Patients with neuromuscular disease Patients with pneumothorax Patients with diaphragmatic paralysis.
