Eligibility Criteria
Inclusion Criteria
- •1\.Prefrail or frail older patients age 60 years or older
- •2\.Visit geriatric clinic Siriraj Hospital or Ratchapiphat Hospital
- •3\.lives with caregiver
- •4\.able to access internet
Exclusion Criteria
- •1\.Patients who cannot communicate
- •2\.Cognitive impairment ( TMSE \<18\)
- •3\.Life expectancy \<6 months
- •4\.Unstable condition ( Congestive heart failure, Cardiovascular or respiratory failure , Blood pressure \<90/60mmHg, or \>180/100mmHg, heart rate \>100bpm, oxygen saturation \< 92%, Respiratory rate 22/min)
- •5\.disabled people
- •6\. people with prosthesis which disturbs body composition measurement by bioelectrical impedance analysis
- •7\.History of hospitalization within 3 months
