Maori (self-identified)
Aged 55 years or older
At 'high risk' of healthcare utilisation (has calculated from general practice prioritisaton tools embedded in patient management software).
Enrolled in one of the study general practices
Exclusion Criteria
•Unable to provide informed consent AND/OR
•Living in an Aged Residential Care facility
