Inclusion Criteria
heart failure patient without any information about their disease and with out any source like doctor or nurse to gain information; literate ability to read and write; patient between 18 to 85; patient who want will included in this study
Exclusion criteria:illiterate persons; informed persons; unconscious persons; chronic kidney patients; dialysis patients; brain patients

Name Time Method Rate of readmission. Timepoint: 3, 6 and twelve month after initialization of intervention. Method of measurement: contact and questionnaire.

Name Time Method Treatment Expenses. Timepoint: Six and Twelve months after finalization of intervention. Method of measurement: contact-questionnaire.;Probable side effect(like:stress) after presentation of physician prescribed information. Timepoint: Twelve months after finilization of intervention.
