Inclusion Criteria

  • •\- Elderly, 65 years of age and older
  • •\- Community\-dwelling
  • •\- Referral from the general practitioner due to chronic condition
  • •\- FAC 3 or more

Exclusion Criteria

  • •If an older adult is insufficiently instructible after assessment by the physical therapist, he or she can not participate in this study.
