- •1\) Synchronous other cancer 2\) Female with present pregnancy or breast\-feeding, or contemplating pregnancy 3\) Mental disorders which may affect ability or willingness to provide informed consent of the study 4\) Continuous systemic steroid therapy 5\) Interstitial pneumonia, pulmonary fibrosis, or severe emphysema 6\) Active bacterial or fungal infection 7\) Past history of myocardial infarction or unstable angina within 6 months 8\) Uncontrollable hypertension
