General practitioners & practice assistants: Established, licensed general practitioners registered in Baden-Wuerttemberg or North Rhine-Westphalia and their practice assistants who are of legal age and able to consent.
Dentists & dental practice assistants: Established, licensed dentists registered in Baden-Wuerttemberg or North Rhine-Westphalia and their practice assistants who are of legal age and able to consent.
Patients in the general practitioner study: Participants of legal age in the type 2 diabetes disease management program who have been participating in the DMP for at least one year and who did not receive a periodontitis-treatment within the past 12 months. All patients need to be insured in the statutory health insurance. Since the control group is calculated from medical claims data, no patients will be recruited for the control group.
Patients in the dentists study: Patients of legal age who require a systematic periodontosis treatment and who were not yet diagnosed with type 2 oder type 1 diabetes. All patients need to be insured in the statutory health insurance. These criteria are applicable for both the intervention and the control group.
Physicians and practice assistants in both studies are not eligible for participation if they:
- live in a nursing home
- are under legal guardianship
- are not of legal age
- are cognitively and/or verbally incapable of participating in the study
- are unlicensed physicians
- their practice is not registered in Baden-Wuerttemberg or North Rhine-Westphalia
Patients in both studies are not eligible for participation if they:

- do not have health insurance in Germany
- may not claim services from the statutory health insurance that exceed basic coverage despite them being insured (e.g. due to legal regulations or unpaid fees)
- Additionally in the general practice study: Patients who have not been participating in the type 2 diabetes DMP for at least the past year as well as patients who received periodontitis treatment within the past 12 months.
- Additionally in the dentist study: Patients who are already diagnosed with diabetes.

Name Time Method Primary outcome general practitioner study: Reduction of the HbA1c-parameter in patients who participate in the disease management program for type 2 diabetes and who did not receive a periodontosis treatment within the past 12 months.<br><br>Primary outcome dentist study: Rate of new diabetes diagnoses within 12 months in patients suffering from pariodontosis who have not been diagnosed with diabetes before.
