Patients who have been admitted to the AGCH can be included in the study, admission is based on following criteria:
1. Patients requiring hospital admission for both a common medical problem as a geriatric problem;
2. 65 years and older;
3. Living in the catchment area around the Amsterdam UMC, location AMC.
4. Not requiring additional diagnostics or hospital treatment that cannot be provided at the AGCH.
Patients are excluded from the study if: 1) the attending physician judges that the patient is too ill to participate e.g. is terminally ill 2) the patient or legal representative does not consent to participate. 3) the patient or legal representative does not speak or understand Dutch or English.

- Primary Outcome Measures
Name Time Method The primary outcome measure is the 3-month unplanned readmission rate to the AGCH or hospital.
- Secondary Outcome Measures
Name Time Method 1) ADL-functioning as defined by the KATZ-ADL scale. <br>2) Institutionalization in a nursing home or long-term care facility. <br>3) Health-related quality of life (HRQoL) <br>4) Satisfaction of patients and primary care givers with the care provided. <br>5) Health care utilization, including institutionalization (temporarily or permanent), hospitalization (plannend and unplanned), home care, physiotherapy, emergency department, primary care use and informal care hours. <br>6) All-cause mortality
