- •1. Age: Children aged 6-<12 y, adolescents aged 12-<18 y, parents of paediatric patients aged 0-<18 y, health care professionals (nurses and
- •paediatricians) taking care of paediatric patients aged 0-<18 y
- •2. Sex: For paediatric patients only: male or female
- •3. Health: For paediatric patients only: acutely or chronically sick. For parents and health care professionals: taking care of acutely or
- •chronically sick children/adolescents the definition by Mokkink LB et. al, a participant is considered to be chronically sick if the underlying disease or condition lasts 3 months or more or has occurred 3 times or more during the past year and will probably reoccur. It affects a child’s normal activities and requires frequent hospitalisations, home health care, and/or extensive
- •medical care. The underlying disease or condition is not (yet) curable or is
- •highly resistant to treatment.
- •4. Compliance: Participant must understand and be willing, able, and likely to comply with the survey procedures. Participant must be able to understand the questions
- •5. Consent: Participant has verbally confirmed his/her consent, documented with a cross on the CRF by the investigator. For children and adolescent: the parent has verbally indicated his/her consent, documented with a cross on the CRF by the investigator
- •6. Affiliation: Participant must be either patient or parent of a patient at the Department of General Paediatrics, Neonatology and Paediatric Cardiology of the University Hospital Düsseldorf Or Participants must be a health care professional (nurse or paediatrician) employed at the Department of General Paediatrics, Neonatology and Paediatric Cardiology of the University Hospital Düsseldorf

- •For paediatric patients aged 6-<18 y: not fully oriented, e.g. in the post-operative period and not yet fully awoke
