入选标准
- •Aged 70 years or older; receiving medication from the public pharmacy; use of 5 or more different medications; chronic use of antidiabetic (at least 1 medication next to metformin) and/or cardiovascular medication; HbA1C < 54 mmol/mol; systolic blood pressure < 140 mmHg.
排除标准
- •Not having signed the statement of consent; unable to fill out questionnaires; not able to speak, write, or understand the Dutch language; life expectancy < 3 months; participation in other research projects.
